Photo ID: Just a picture card that shows it's you! Insurance Card: If you have one, bring along your insurance card. List of Medications: If you take any medicines, make a list or just pack them up – whatever's easier for you. HSA Card: If you've got an HSA card, bring it along, too.
And guess what? We have some forms for you, but don't worry, they're simple! You can find them on our website. Just print them out, fill them in, and bring them to your first appointment. Or, if you prefer, you can even email them to us at info@pfmed.net.

Want a little tip to speed things up? The verification process will be a breeze if you give us a copy of your insurance card before your appointment. And if you bring your card to the appointment and we can confirm your insurance, you'll be like a self-pay patient until we get it all sorted out.
